Silent sinus syndrome (SSS) is a rare condition characterized by progressive enophthalmos and hypoglobus due to spontaneous collapse of the maxillary sinus walls and orbital floor descent. Due to its rarity, significant controversies persist regarding its etiology, diagnosis, and optimal management strategies. We present a rare post-traumatic SSS case after a non-displaced orbital fracture, successfully treated with a single-stage procedure combining endoscopic sinus surgery and orbital reconstruction using a patient-specific orbital implant (PSI) designed via a novel virtual surgical planning workflow. Bone remodeling in SSS differs from post-traumatic cases, presenting challenges in orbital reconstruction. PSIs, used with endoscopic sinus surgery, are safe and allow precise restoration of orbital anatomy. This case demonstrates the efficacy of PSIs in post-traumatic SSS management, highlighting the advantages of computer-assisted planning and titanium PSIs as the preferred approach for precise orbital reconstruction and optimal outcomes.
