Midfacial reconstruction represents a challenge because of the complex anatomy and essential functions of the region, including dental support, orbital integrity, nasal airflow, speech, and facial esthetics. Various reconstructive options exist, ranging from prosthetic obturators to complex microsurgical free flaps. Recent advances in digital planning and custom manufacturing offer promising solutions and this clinical report describes a primary, one-step complete maxillary reconstruction combining a free fibula flap with a custom subperiosteal implant for a patient with an ossifying fibroma. At 12 months follow up, this combined approach demonstrated feasible and effective results, particularly for patients with benign tumors.
