The authors describe a very unusual oblique scar up to the shoulder resulting from a previous radical mastectomy. A simple method to reconstruct or enhance the anterior axillary fold with a turnover dermal-fat flap during nipple-areola-complex reconstruction is reported. This surgical procedure was performed in a patient with a pedicled TRAM-flap breast reconstruction. The subcutaneous dermal-fat flap, based on the dermis and fat of the dog-ear resulting at the lateral end of the previous mastectomy scar, was de-epithelized and turned over to reconstruct the anterior axillary fold. The overlying skin was exploited as a graft to reconstruct the areola. Indications for a turnover dermal-fat flap in anterior axillary fold reconstruction are discussed.

Abstract
